Area Agency On Aging of NW MI Care Connection Programs

The Care Connections programs are: Information and Assistance...Care Management...MI Choice Waiver...Caregiver Respite Program...Nursing Facility Transition Services (NFT).

There are several options within the Care Connections program for the individual who chooses to receive long-term care services while living in a community setting. The types of services provided can supplement assistance the individual may already be receiving through an informal network of family and friends or through private purchase.
 
What is Care Connections? It is the assessment, planning and coordination of in-home services by a Registered Nurse and/or Social Worker, (Supports Coordinators), for medically eligible adults.
 
Who is eligible? Adults who are 18 years of age and older with medically complex and diverse service needs. Target groups are those who are eligible for Nursing Facility admission or some other type of assisted living setting.
 
What services are available? Care Connections can supplement a plan of care with respite, personal care, homemaking, private duty nursing, personal emergency response units, and adult day care. Community services such as home delivered meals, Medicare skilled nursing, and therapy services are utilized as well.
 
Who pays for the in-home services?: Services are provided by a variety of different sources including family and friends, community services such as those offered by local Commissions or Councils on Aging, privately paid caregivers,  Medicaid and/or funding from Area Agency on Aging of Northwest Michigan (AAANM). Other payment sources are utilized before AAANM Care Connections funding is used.
