Foundation for Mental Health Permanent Supportive Housing

The Foundation for Mental Health provides affordable housing linked to voluntary support services for people with special needs (mental illness, substance abuse disorder, HIV/AIDS) who are homeless and meet financial eligibility.


Application Process:

Referrals are generally received through case managers. FMH completes a 45 minute intake interview with potential tenant and verifies homelessness, special need, and income. To assist someone with rental subsidy in an apartment we need eligible referral, available subsidy (from our 5-6 HUD contracts/shp leasing) and vacant/available property. Contact FMH with questions re referral process or current capacity to assist new referrals. 929-1309. We do not take applications online.
